How to make an appeal to the Consumer Service Center of the Taipei City Government?

According to Article 2 of the Regulations on Handling an Appeal in a Consumer Dispute issued by the Executive Yuan, a local consumer service center shall accept an appeal when it is in written form; in other words, the plaintiff has to file a written appeal. If the plaintiff files an appeal in person, the service center should acquire the plaintiff’s basic information, the reason for and the intent of the appeal, and the plaintiff’s signature or seal on the written form after all the required information is filled in.
Moreover, the plaintiff has to fill in his/her name, telephone number, current address, the nature of the dispute, the name and the address of the business owner, etc., so that the Consumer Service Center of Taipei City Government can handle the case.
