- You can visit the website of the Department of Legal Affairs of the Taipei City Government and click on "Froms Download" to download the "State Compensation Application Form" under the heading "Online Service". Upon completing the form, mail it to the compensating authority.
- You can also click on "On-line claim for state compensation" to file a state compensation claim on line.
- After completing the claim form for state compensation, you must attach the relevant documentation to the compensating authority for compensation. Items which need to be filled in on the claim form include:
- The claimant’s name, sex, birth date, birth place, identification number, occupation, and current address or permanent address. If the claimant is a corporate body or group, its name and major office or major business office, as well as the name, sex, current address or permanent address of the representative of the corporate or group.
- If there is an agent, the name, sex, birth date, birth place, identification number, occupation, current address or permanent address of the agent.
- Relevant details and the reason for claiming for state compensation.
- The total amount of compensation for damage claimed for or the details for the restoration of the damage.
- The compensating authority.
